As a patient, you have the right:
- To respectful and considerate care.
- To privacy.
- To a clear, complete, and understandable description of your condition and treatment choices.
- To ask questions and expect answers about benefits and risks before you give your permission for any procedure or treatment.
- To refuse a diagnostic or therapeutic procedure or treatment.
- To access your medical record.
- To contact the office manager if you have concerns or complaints about the care you receive.
- To leave the Medical Center as soon as possible with instructions about caring for yourself at home.
- To change primary or specialty physicians if other qualified physicians are available.
As a patient, you have the responsibility:
- To give your health care team the most complete and correct information about your health, health history, insurance, and related issues.
- To tell your caregivers about changes in the way you feel when you are in the Medical Center, or after you leave.
- To follow your plan of care.
To be considerate and respectful of other patients and Medical Center employees as well as other's property and equipment.
